This is a private service that is provided only within certain catchment areas around Australia. As such, Geriatric Care Australia (GCA) charges an upfront fee for these appointments where the patient receives a Medicare reimbursement.

A New Patient Form and a GP Referral must be obtained prior to completing this form. Once obtained, complete the below payment form and the GCA Care Coordination Team will reach out within 3 business days to schedule a time for the consulting geriatrician to provide a review in the home. We encourage family and next of kin to attend appointments with patient consent.
